  New MCP primitives for surgical note edits (brain#4), contained to the
  sync source with governance files behind a de-surfaced, principal-gated
  door. Closes a pre-existing brain_get exposure that returned verbatim
  session transcripts by path.

  Also: frontmatter lint + repair (#90), rune-safe truncation (#93),
  OIDC discovery retry on cold boot (#83), aging review-queue nudge (#82),
  brain_index de-surfaced (#78), and I1 refusals that name what escalated
  the classification (#91).
