fix(language): LiteLLM gateway never-say — stale endpoints, not piguard
piguard is legitimately in the request path as the reverse proxy; only the stale endpoint forms (piguard:4000, koala:4000) are wrong. Per brain#6 review amendment #3 / tapir#18 follow-on.
